PRICE PROPOSAL FORM

The fair market rent for the Mordecai Lincoln Cottage is $2,250 per month. Payments are per

month for twelve (12) months, with the option to renew for up to two additional terms in one-year

increments at an increase of 3% each year by mutual consent.

Proposer agrees to provide the services outlined in this RFP.
