Project Description: Under this RFP, Clinical Chart Abstraction Services is defined as the process of collecting demographic and clinical information from a patient’s medical record, evaluating data relative to measure specifications and criteria, transcribing appropriate data to the clinical data abstraction or registry platform to ensure complete and accurate data are submitted to regulatory, accrediting, and benchmarking entities, supporting CMS validation activities, and providing feedback to the MSRH on performance improvement opportunities.
