MIDDLETOWN AREA SCHOOL DISTRICTREQUEST FOR PROPOSAL FOR SUPPLEMENTAL CERTIFIED ATHLETIC TRAINER SERVICESIssued: May 8, 2026Intent.It is the intent of the Middletown Area School District (the "District") to secure proposals for SupplementalCertified Athletic Trainer services for the Middletown Area High School and the Middletown Area MiddleSchool, located in Middletown, PennsylvaniaContract Term.The Contract shall begin on July 1, 2026, and run for a 36-month term, expiring June 30, 2029.Due Date and Submission of Proposals.All proposals are due by 1:00 p.m., on Thursday, May 28, 2026. Proposals must be mailed to:Middletown Area School DistrictAttn: David Franklin55 West Water StreetMiddletown, PA 17057Proposals shall have "Supplemental Certified Athletic Trainer Services Proposal" on the outside of theenvelope. The District reserves the right to reject any and/or all proposals and to waive, at its discretion,any irregularities, mistakes, omissions, or informalities.Questions should be directed to Eric Graeff, Procurement Manager, at egraeff@raiderweb.org.Prospective vendors will find all questions and answers posted on our website. The final day for questionswill be the close of business on May 20, 2026 , all questions and answers will be posted on our websiteon May 22, 2026.Agreement.Prospective Providers shall include a copy of their proposed agreement with their proposal submission.Final agreement shall be as mutually agreed, and shall incorporate the scope of services and conditionsas herein described.Selection Criteria.Page 1 of 5The selection criteria shall include, but not be limited to, cost, Athletic Trainer experience, references,agreement language, and insurance coverage. The District reserves the right to select the Provider whobest meets its needs.Notice.As this Request for Proposal is not a competitive bid, the District retains the right to negotiate the priceand other terms of the contract with the respondents identified and chosen by the District.Insurance Requirements.Please see attached addendum.Independent Contractor.It is hereby understood and agreed that the Provider, in performing this Agreement, is acting in the capacityof an independent contractor and that the Contractor is not an agent, servant, partner, nor employee ofthe District. The Contractor will have control over the work to be performed, provided same is consistentwith the requirements of the Agreement, and shall be solely responsible to pay its own federal, state, andlocal taxes, salaries, Social Security payments, and any and all other payments incurred by the Contractorin the performance of this Agreement, as well as perform all necessary legal requirements pertaining t oemployment. None of the benefits provided by the District to its employees including, but not limited to,workers' compensation insurance, disability insurance, medical insurance, and employment insurance areavailable from the District to the Contractor and/or any and all of the Contractor's agents, servants, andemployees. The Contractor has no authority hereunder to assume or create any obligation or responsibility,express or implied, on behalf or in the name of the District or to bind the District in any way whatsoever.SCOPE OF SERVICE & REQUIREMENTSThe Provider shall provide an Athletic Trainer who is certified by NATA BOC AND possesses a valid AthleticTraining License in the Commonwealth of Pennsylvania. The School District shall have the right to consentto assignment of the Athletic Trainer. All Athletic Training Services provided under this Agreement shall beperformed exclusively for and on behalf of the School District. The Provider, as per availability of Provider'sStaff, will:a. Part Time (approximately 800 hours per year) or Full Time (approximately 1400 hours per year) trainingservicesb. Assist the team physician(s) in maintaining an appropriate medical records system.c. Monitor athletic injuries and develop injury prevention programs with the advice and consent of theteam physician(s).d. Coordinate injury evaluation and provide injury follow-up with the team physician(s) on a regular basis.e. Operate in accordance with rules and regulatioPnasgoef2thoef 5Commonwealth of Pennsylvania.f. Coordinate all Athletic Training Services on an as needed basis at the direction of the School District'sAthletic Director to supplement the services provided by School District's athletic trainer as per availabilityof Provider's Staff.g. Provide home and away Athletic Training services during playoffs on an as needed basis.h. Create documentation of the Athletic Training Services provided to any injured student-athlete. Inproviding Athletic Training Services to injured student-athletes, the Athletic Trainer(s) shall act solely andexclusively on behalf of the School District. All documentation and treatment notes shall be subject to The FamilyEducational Rights and Privacy Act and privacy regulations promulgated thereunder ("FERPA") and may bedisclosed to the School District, school officials, coaches, athletic directors, team physicians, nursesand other school employees without authorization. The School District shall retain full control and ownership ofthe records created under this Section.Coverage.Part Time Athletic training coverage is estimated to be 800 total hours for each year. Full Time AthleticCoverage is estimated to be 1400 total hours for each year. Not to exceed 40 hours per week.Provide athletic training coverage for students in grades 7-12 participating in PIAA sports at Middletown AreaSchool District. Traditional athletic training hours to include afternoons and evenings with occasional weekendcoverage and full day coverage during preseason or school breaks as needed. Schedule to be determined by thedistrict’s athletic director and athletic trainer and may include preseason, regular season, and post-season play bydistrict teams. Travel may be required for varsity football games and playoff competitions in all sports asnecessary• Fall Season - Coverage by Athletic Trainer during pre-season football camp; coverage of all home andaway Varsity football games; coverage of all Junior Varsity and freshman home games only; on-sitefor all home Varsity sports contests; on-site for weekday afternoon practices and weekend practiceavailability as determined and set forth by the Athletic Director. Attendance at scheduled nightgames shall take precedence over attendance at afternoon practices.• Winter Season - Coverage of all home basketball games and wrestling matches; on-site for weekdayafternoon practices and weekend practices as determined and set forth by the Athletic Director.• Spring Season - Coverage of all home Varsity sports contests; on-site for weekday afternoonpractices and weekend practices as determined and set forth by the Athletic Director.• Summer - Coverage from the first day of football heat acclimatization and practices.Actual schedule shall be confirmed and administered by the District Athletic Director.Page 3 of 5DEVIATIONSThe requirements contained in the solicitation become part of the terms and conditions of the contract resultingfrom this solicitation. Any deviations from the solicitation in must be clearly defined by the vendor in its proposaland, if accepted by the agency, will become part of the contract.Deviations must not conflict with the basic nature of the solicitation, requirements, or applicable state or federallaws. Middletown Area School District reserves the right to accept or reject any deviation.Bidders must submit all deviations in writing below.
