ADDITIONAL FORMSATTACHMENT A – GENERAL PRICING SHEETPAGE3338888910111417262|PageRFP 3192ON-CALL PLUMBING SERVICESI.PURPOSEThe purpose of this Request for Proposal (“RFP”) is to solicit sealed proposals through competitivenegotiation for On-Call Plumbing Services for the School Board of the City of Roanoke, morecommonly known as Roanoke City Public Schools. Roanoke City Public Schools (“RCPS”, “Owner”,or “Division”) intends to retain a qualified firm(s) (“Contractor”,” Offeror”) to provide these servicesfor the Division. RCPS reserves the right to make an award to one or more qualified firms.RCPS will select the Contractor, or Contractors, who, in the aggregate, most completely meet theneeds of the Division.This document establishes the anticipated services to be performed and outlines the evaluation andselection process. However, this document does not guarantee a contract.II. BACKGROUNDRoanoke City Public Schools (“RCPS”) is a progressive urban school district serving the City ofRoanoke, Virginia. The district is comprised of seventeen elementary schools, five middle schools,two high schools, the Roanoke Valley Governor’s School for Science and Technology, the Noel C.Taylor Learning Academy, Forest Park Academy (which serves overage and under-credited middleschool and high school students), adult education programs, career and technical educationprograms, preschool programs for low-income families, and after-school programs to assiststudents needing additional instruction.RCPS has approximately 13,700 students and approximately 2,200 employees.III. STATEMENT OF NEEDA. Contractor General Responsibilities: The Contractor shall provide all labor, supervision,equipment, tools, materials, and incidentals required to completely and satisfactorily performvarious plumbing service projects as identified by RCPS’ representative. Services include, butare not limited to, the following:1. Troubleshoot, repair, and replacement of domestic water, sanitary sewer, and storm pipingsystems.2. Repair, replacement, and installation of plumbing fixtures, including toilets, urinals, sinks,faucets, valves, drinking fountains, and hose bibs.3|Page3. Repair, installation, and replacement of water heaters, circulating pumps, expansion tanks,mixing valves, and related components.4. Clearing of drain, waste, and vent (DWV) piping using mechanical or approved methods.5. Repair, installation, and replacement of underground and above-ground piping systems(copper, PVC, CPVC, cast iron, ductile iron, PEX, etc.).6. Emergency response for water leaks, line breaks, flooding, or system failures.7. Repair, installation, and replacement of natural gas, propane, and hot water/steam piping(hydronic heating).8. Limited design assistance related to sizing, routing, and material selection for piping andplumbing components.8. Coordination with RCPS staff to minimize service interruptions during instructional hours.B. Contractor Performance Responsibilities: At a minimum, the Contractor should meet thefollowing performance standards.1. Unless an emergency exists, the Contractor should respond to RCPS’ request forservice within 24 hours of notification.2. The Contractor shall provide RCPS with a written non-binding estimate of thepromulgated work and receive written approval to proceed (the “Notice to Proceed”) beforeinitiating any work. The estimate and Notice to Proceed shall be considered an addendum toany contract resulting from this RFP. At a minimum, the Contractor’s non-binding estimate shallinclude the number of labor hours needed for each defined grade of worker, start date andtime of work, estimated duration of time the Contractor is occupying the job site, and thematerials required for the work. The Contractor shall not charge RCPS for their time inspectingthe work site or for developing the written non-binding estimate. Contractor shall review safetyplans with RCPS prior to beginning any work. The Contractor shall coordinate all work with RCPSto minimize site disturbance and service impacts.3. The Contractor shall begin work within three (3) business days of the issuance ofa Notice to Proceed, except in cases of emergency where work must begin within twenty-four(24) hours, or unless otherwise scheduled with RCPS approval. Contractor shall ensure that anyrequired dig alerts are completed and the area marked by Miss Utility in advance of performingany digging. Where digging is required, the Contractor shall contact Miss Utility within 24 hoursof issuance of the Notice to Proceed.4. The Contractor shall possess the appropriate contractor’s license with thespecific specialty services as recognized by the Virginia Board for Contractors, Department ofProfessional and Occupational Regulation, to perform plumbing services.4|Page5. The Contractor shall be responsible for providing quality plumbing services inaccordance with industry standards, and follow any plans, directions, and instructions suchas provided by RCPS’ representative for each task.6. The Contractor shall be solely responsible for obtaining any necessary permits toperform any particular task.7. All equipment, materials, and installation work provided shall conform to theVirginia Uniform Statewide Building Codes, VOSHA, and the specifications of this solicitation.8. Time charged for a task shall ONLY be for the actual time spent on the job. Traveltime, break time, and overhead costs shall not be billable for any task andincluded within the Contractor’s loaded hourly rate.9. Invoices for materials provided by the Contractor shall be solely limited to theactual invoice cost for the materials or equipment provided by the Contractorand used for the task. The Contractor is prohibited to add or bill RCPS with anyoverages, up-charges, or cost-plus charges for any material or equipment. Thecosts of consumable materials used to complete the work shall be included inthe contract labor rates. All costs shall be included in the work authorization andreceipts showing vendor purchased, date, description, and cost are required formaterial reimbursement. For multi-day projects, RCPS may request that a dailylog be submitted which includes details concerning the completed work, labor,materials costs.10. The Contractor shall provide a clear and legible copy of an invoice showing allwork performed, indicating the time or arrival and departure at RCPS facility foreach employee performing services, including copies of any material orequipment invoices that denote the Contractor’s cost. The Contactor shallcontact RCPS’ representative at the completion of work in order to verify thetime allotted to the service.11. Contractor shall report any safety concerns, near misses, accidents, and injuriesrelated to the work area to the RCPS immediately.12. Contractor shall warrant replace or repair any defective product, materials,Installation, or services for one (1) year from RCPS acceptance of any serviceshereunder, work or services at no additional cost. RCPS shall be deemed to haveaccepted services hereunder only after receipt of a proper and detailed invoicefrom Contractor for such services and payment by RCPS to Contractor in full ofsuch invoice.13. Contractor shall respond to emergency warranty repairs within one (1) day andshall respond to routine warranty repairs within three (3) business days.5|Page
