EXHIBIT A - SCOPE OF WORKSCOPE OF WORKAmerica 250 Monument1. A qualified licensed contractor” (hereinafter “Contractor”) shall furnish all labor, materials,equipment, component/devices, transportation, fuel, supervision, permits, inspections, and allother incidentals necessary to complete all necessary work listed, all in accordance with allparts of this solicitation and bid documents provided for the America 250 Monument, to beinstalled at the Lake County Historical Court House (Museum), located at 317 West MainStreet, Tavares, FL 32778.2. Provide and install a granite monument with a bronze memorial plaque on a new concretemonument base (footing) covered with pavers as generally depicted in Exhibits attached to thissolicitation.2.1. Materials to be used for the monument are a smooth gray granite and a bronze plaque.2.1.1. Smooth Gray Granite: Refer to attached photos of existing monument, theproposed America 250 Monument will be similar in size, shape and form.2.1.2. Bronze Plaque: Size 2’ wide by 3’ high. Refer to attached CommemorativeMonument Content for art and raised text to be included in the bronze plaque.Contractor shall submit a draft to Lake County Facilities staff for review/approvalbefore proceeding with the manufacturing of the bronze plaque.2.2. Rough Dimensions (Refer to attached Exhibit with Photos of Existing Monument identifyas “Similar Existing Monument Details”):2.2.1. Height is approximately four feet (4’);2.2.2. Width is approximately three feet (3’); and2.2.3. Depth at the base is approximately two feet (2’).2.2.4. Refer to Exhibit for additional monument dimensions and details.2.3. Granite monument may be constructed of several pieces.2.4. Contractor to demolish, remove and dispose existing monument concrete base (footing)in a legal and proper manner, and replace with a new minimum 5' long x 4' wide x 1’4"deep concrete base (footing) with #5 reinforced bars at 14” spacing each way. Concretebase to be covered with pavers similar to attached photo of existing monument.*Note: Contractor is informed that there is an existing irrigation lines and sprinkler headslocated within the proposed monument area that needs to be protected during construction,refer to Exhibit - Proposed Monument Location. Contractor responsible for identifying allutilities before starting construction. Contractor to inform Lake County Facilities staffwith enough notice if there is a need to relocate any existing utilities. Lake County will beresponsible for relocating any existing utilities as necessary for the construction of theconcrete base. Contractor must take every precaution to avoid damage to any undergroundutilities. Contractor will be responsible for any damage caused to the existing utilities. Inthe event that Contractor or any of its subcontractors damage an existing utility, theContractor will be responsible for the repair and must repair the same at its own cost.2.5. Lake County Parks and Water Resources, Facilities Management will be providing theBuilding Permit (Plan Review Only) for the proposed America 250 Monument, however,Page 1 of 4EXHIBIT A - SCOPE OF WORKAmerica 250 Monumentthe Office of Building Services has indicated Contractor will be required to complete aBuilding Permit Application prior to issuance of this Permit. In addition, the Contractorand any subcontractors must be added to the building permit and pay all related permitand inspection cost/fees for the required work included under this solicitation.3. DELIVERYAfter selection of sign and artwork has been approved by the County and issuance of noticeto proceed has been received, the completion shall be no later than June 26, 2026.4. ALTERNATE OFFERS MAY BE CONSIDEREDThe County will consider one (1) alternate offer from a vendor which has submitted aprimary offer for this solicitation; provided that the vendor of record for both the primaryand alternate offer is the same entity, and that the alternate offer provides a different productor level of service that meets or exceeds the contract requirements. For the County to consideran alternate offer, the vendor shall provide a complete and separate pricing section using theforms set forth within this solicitation and shall mark “Alternate Offer” on the first page of thealternate pricing section. The County reserves the right to consider and award, based on theCounty’s best interests, any alternate offer based on the method of award prescribed in thissolicitation.5. ACCEPTANCE OF GOODSThe product(s) delivered as a result of an award from this solicitation shall remain the propertyof the Contractor, and services rendered under the contract will not be deemed complete,until a physical inspection and actual usage of the product(s) and/or service(s) is (are)accepted by the County and shall be in compliance with the terms herein, fully in accordwith the specifications and of the highest quality.In addition, any defective product or service or any product or service not delivered orperformed by the date specified in the purchase order or contract, may be procured by the Countyon the open market, and any increase in cost may be charged against the awarded Contractor.Any cost incurred by the County in any re-procurement plus any increased product or servicecost shall be withheld from any monies owed to the Contractor by the County for any contractor financial obligation.6. ACCIDENT PREVENTION AND BARRICADESPrecautions must always be exercised for the protection of persons and property. AllContractors performing services under the contract must conform to all relevant Federal,State and County regulations during such effort. Any fines levied by the above-mentionedauthorities for failure to comply with these requirements will be borne solely by theresponsible Contractor. Barricades must be provided by the Contractor when work isperformed in areas traversed by persons and when deemed necessary by the County ProjectManager.7. BUSINESS HOURS OF OPERATIONNo work may be done on Saturday, Sunday, or on weekdays between the hours of 5:00 P.M.and 7:00 A.M. except when necessary for the proper care and protection of work alreadyperformed, and when permission is secured from the County Project Manager.Page 2 of 4EXHIBIT A - SCOPE OF WORK8. CLEAN-UPAmerica 250 MonumentAll unusable materials and debris shall be removed from the premises at the end of eachworkday and disposed of in an appropriate manner. Upon final completion, Contractor shallthoroughly clean up all areas where work has been involved as mutually agreed with theCounty Project Manager.9. CONFLICTS BETWEEN THE DRAWINGS AND SPECIFICATIONSIn the event of any conflict between the drawings and specifications contained within thecontract, the following guidance will govern:A. Addenda will supersede all other Contract Documents to the extent specified with theAddenda. Subsequent addenda will supersede prior addenda only to the extent specified.B. Drawings and specifications are intended to agree and be mutually complete. Any itemnot contained within the drawings, but contained in the specifications, or vice-versa, must beprovided or executed as shown in either the drawings or specifications at no extra cost to theCounty. Should anything not included in either the drawings and the specifications benecessary for the proper construction or operation of the work specified in this solicitation, orshould any error or disagreement between the specifications and drawings exist or appear toexist, the Contractor shall not derive unjust benefit thereby, or use such disagreement counterto the best interests of the County. If the discrepancy is noted before initial submission ofoffers, or before contract award, the Contractor shall immediately advise the designatedprocurement agent of the perceived discrepancy. If the discrepancy is noted after award, theContractor shall immediately advise the County Project Manager, and await that individual’sresultant direction, before proceeding with the work in question.10. OMISSION FROM THE SPECIFICATIONSThe apparent silence of this specification and any addendum regarding any details, or theomission from the specification of a detailed description concerning any point, will be regardedas meaning that only the best commercial practices are to prevail, and that only materials andworkmanship of first quality are to be used. All interpretations of this specification will bemade upon the basis of the contract.11. DEFICIENCIES IN WORK TO BE CORRECTED BY THE CONTRACTORThe Contractor shall promptly correct all apparent and latent deficiencies and/or defects inwork, and/or any work that fails to conform to the contract documents regardless of projectcompletion status. All corrections shall be made within five (5) calendar days after suchrejected defects, deficiencies, and/or non-conformances are verbally reported to theContractor by the County's project administrator, who may confirm all such verbal reportsin writing. The Contractor shall bear all costs of correcting such rejected work. If theContractor fails to correct the work within the period specified, the County may, at itsdiscretion, notify the Contractor, in writing, that the Contractor is subject to contractualdefault provisions if the corrections are not completed to the satisfaction of the County withinfive (5) calendar days of receipt of the notice. If the Contractor fails to correct the workwithin the period specified in the notice, the County shall place the Contractor in default,obtain the services of another Contractor to correct the deficiencies, and charge the incumbentContractor for these costs, either through a deduction from the final payment owed to thePage 3 of 4EXHIBIT A - SCOPE OF WORKAmerica 250 MonumentContractor or through invoicing. If the Contractor fails to honor this invoice or credit memo,the County may terminate the contract for default.12. FURNISH AND INSTALL REQUIREMENTSThe specifications and statement of work contained within this solicitation describe thevarious functions and classes of work required as necessary for the completion of theproject. Any omissions of inherent technical functions or classes of work within thespecifications or statement of work will not relieve the Contractor from furnishing, installing orperforming such work where required for the satisfactory completion of the project.13. PROTECTION OF PROPERTYAll existing structures, utilities, services, roads, trees, shrubbery, and property in which theCounty has an interest shall be protected against damage or interrupted services at alltimes by the Contractor during the term of this contract; and the Contractor shall be heldresponsible for repairing or replacing property to the satisfaction of the County which isdamaged by reason of the Contractor’s operation on the property. In the event the Contractorfails to comply with these requirements, the County reserves the right to secure the requiredservices and charge the costs of such services back to the Contractor.Page 4 of 4
