SCOPE AND BACKGROUND: Through this Request for Proposal (RFP) the Agency of DigitalServices, on behalf of the Green Mountain Care Board, (hereinafter the “State”) is seeking toestablish contracts with one or more companies that can provide Health Data InfrastructureTransformation Support.1.2. CONTRACT PERIOD: Contracts arising from this RFP will be for a period of two years with anoption to renew for up to three additional twelve-month periods. The State anticipates the startdate for such contract(s) will be August 2026.1.3. SINGLE POINT OF CONTACT: All communications concerning this RFP are to be addressed inwriting to the State Contact listed on the front page of this RFP. Actual or attempted contact withany other individual from the State concerning this RFP is strictly prohibited and may result indisqualification.1.4. BIDDERS’ CONFERENCE: A non-mandatory bidders’ conference will be held remotely at the dateand time indicated on the front page of this RFP.1.5. QUESTION AND ANSWER PERIOD: Any bidder requiring clarification of any section of this RFP orwishing to comment on any requirement of the RFP must submit specific questions in writing nolater than the deadline for question indicated on the first page of this RFP. DETAILED REQUIREMENTS/DESIRED OUTCOMESPlease see Attachment 7.6 for details related to the Rural Health Transformation Grant that is funding thisproject.2.1. BackgroundThe State of Vermont (State) has been awarded federal funding through the 2025 Rural HealthTransformation (RHT) Grant, in part, to support the transformation of the State’s health datainfrastructure. The funds will be distributed over five years. In years one and two, the Green MountainCare Board (GMCB/Board), the State entity responsible for core data (e.g., VUHHDS, VHCURES,etc.) critical to the State’s health data infrastructure, is prioritizing:Revised: February 13, 2026Page 2 of 48• Configuring GMCB’s data infrastructure within the Statewide Data Lake managed by theAgency of Digital Services (“Statewide Data Lake”) (the Statewide Data Lake is for internalState use and enables sharing across agencies),• Modernizing data ingestion from external submitters,• Governing data so that it is stewarded well, of quality, documented, and fit for purpose,• Ingesting major datasets into the Statewide Data Lake, and• Embedding strong project governance, management, communication, and changemanagement practices.The Year 1 and 2 strategy for the RHT initiative is intentionally designed to build a modern, reliabledata infrastructure while simultaneously creating a staffing model that enables embedded teammembers to effectively join the GMCB in this work. By establishing foundational capabilities—suchas modernized ingestion pipelines, entity resolution, robust governance practices, and Agile projectmanagement—the plan ensures that new team members can integrate seamlessly into ongoingoperations and contribute meaningfully from day one.The staffing approach, which includes architects, engineers, governance roles, and changemanagement support, is structured not merely to deliver technical outputs but to embed sharedpractices, transparent workflows, and durable organizational capacity within GMCB. This modelpositions embedded staff to become fully aligned partners in advancing data modernization,strengthening operational agility, and sustaining long-term transformation.Years 3 and following will be executed depending on the availability of funding. Future years andphases of this work will include moderate priority datasets and integration of the new dataenvironment into standard workflows focusing on advancing governance, the enterprise data model,AI-driven automation and analytics, a culture of data literacy, and operational sustainability.The Agency of Digital Services (ADS) is Vermont’s centralized, full-service technology agency. ADShas established a shared responsibility model for data management. Through this procurement,ADS is working to establish an environment that GMCB staff can use to self-serve data management,analysis, and use activities while building on the established common platform and framework andsupported by ADS in the Statewide Data Lake.2.2. Existing Technology EnvironmentThe Board is currently engaged in a project to standardize and improve their existing Azure DevOps(ADO) and Azure Databricks environments by fully integrating them and establishing more matureagile practices and processes.ADO is the organization’s centralized DataOps environment, supporting Agile workflow management,automation, knowledge management, and reporting across governance, data, and operationalactivities.Core Technology Stack:• Azure DevOps: Boards, Repos, Pipelines, Wikis, Analytics• SharePoint: Knowledge and documentation repository• Power BI: Executive and operational dashboards• Power Automate: Process automation and notifications• Teams: Collaboration and ADO integrationRevised: February 13, 2026Page 3 of 48The State has implemented a Statewide Data Lake on AWS, following a medallion architecture andfocusing on native services. The State maintains standard templates that must be utilized to ingestand schedule processing of data.
