Office on Addiction Services Request for Proposals (RFP) for Social Security Income SSI/Social Security Disability Income SSDA Outreach Access and Recovery (SOAR) Case Management 2026

The Mercer County Department of Human Services, Office on Addiction Services, is accepting proposals for the period of one-year (January 1, 2026 – December 31, 2026) with the option to extend for an additional year (January 1, 2027 – December 31, 2027), to provide Social Security Income SSI/Social Security Disability Income SSDA Outreach Access and Recovery (SOAR) Case Management services for indigent Mercer County residents in the amount of up to $200,000.00 annually (State and/or County Funds).
Funding will be made available in the service category:

• Addiction Case Management Services within the Homelessness System
  
  • SOAR Case Manager
    

Eligible applicants must have licensure from the New Jersey Division of Mental Health and Addiction Services (DMHAS). We encourage services to be offered within coordinated networks. Applicant agencies may offer proof of coordination such as consortia or affiliation agreements, letters of endorsement, or other means.
