Project Description:	The City University of New York is a public urban university located in the City of New York and founded in 1847 as the Free Academy. On April 11, 1961, Governor Nelson A. Rockefeller signed the legislation to formally establish CUNY, uniting seven public urban colleges into a formally integrated system. The Senior Colleges of the University are comprised of: Senior Colleges, Graduate and Professional Schools and Other Schools. The following colleges comprise the University: Senior Colleges Bernard M. Baruch College Brooklyn College The City College of New York The College of Staten Island Hunter College John Jay College of Criminal Justice Herbert H. Lehman College Medgar Evers College New York City College of Technology Queens College York College Graduate and Professional Schools The Graduate School and University Center CUNY School of Law The CUNY Graduate School of Journalism CUNY School of Public Health and Health Policy CUNY School of Labor and Urban Studies The CUNY School of Professional Studies CUNY School of Medicine Other Schools The William E. Macaulay Honors College Community Colleges Borough of Manhattan Community College Bronx Community College Eugenio María de Hostos Community College Kingsborough Community College Fiorello H. LaGuardia Community College Queensborough Community College Stella and Charles Guttman Community College The City University of New York (“CUNY” or the “University”) is releasing this Request for Proposals (“RFP”) to solicit proposals from vendors (“Proposers”) to provide an Employee Assistance Program (hereafter “Program”), to be administered exclusively for CUNY’s employees and their families. The Procurement Lobbying Act (PLA) Applies to this Solicitation. The restricted period began with the publication of this ad. Compliance Procurement Lobbying Act Required Forms: Vendor shall complete, sign and submit the following forms if they are selected: 1. “Offeror’s Affirmation of Understanding of and Agreement pursuant to State Finance Law § 139-j (3) and § 139-j (6) (b)” 2. “Offeror’s Disclosure of Prior Non-Responsibility Determinations and Certification of Compliance with State Finance Law §139-j and §139-k” Under the requirements of the State Procurement Lobbying Act all communications regarding advertised projects are to be channeled through the Office of University Controller (*Designated Contacts). Until a designation is made, communication with any other CUNY employees concerning this project that is determined to be an attempt to influence the procurement may result in disqualification. Contact with CUNY: Under the requirements of the PLA, all communications regarding advertised projects are to be channeled through the Designated Contact. Communication with respect to this procurement initiated by or on behalf of an interested vendor through others than the Designated Contact may constitute an “impermissible contact” under NYS law and could result in disqualification of that vendor.
