It shall be the consultants/contractors sole responsibility to comply with allapplicable economic sanctions.Cosumnes Community Services District RFPPre-Employment Psychological Evaluation Services (FD-26-017)SECTION 1 – PURPOSECosumnes Community Services District (“District”) Fire Department (“CFD”) is seekingproposals from qualified providers (‘Proposers”) to provide pre-employment psychologicalevaluation services that meets the District’s needs. The Proposer must have a proven record ofaccomplishment in providing pre-employment psychological evaluation services.SECTION 2 – DISTRICT PROFILEThe District is located in California’s Central Valley in the southern portion of SacramentoCounty, four miles south of the Sacramento City limits. The District is the largest communityservices district in California, encompassing 157 square miles and serving an estimatedpopulation of 210,000.The District is a regional agency providing fire protection services to the cities of Elk Grove andGalt, as well as unincorporated areas in southern Sacramento County. Additionally, the Districtprovides parks and recreation services exclusively to the Elk Grove community.The District is dedicated to enhancing the quality of life of the residents, businesses, visitors,and employees within the diverse community by protecting lives, property, and the environmentthrough superior fire suppression, emergency medical services, fire prevention, and specialoperations response; and by providing parks and recreation services through well-maintainedparks and recreational opportunities for health, wellness, and social interactions.SECTION 3 – SCOPE OF WORKThe District is seeking proposals for pre-employment psychological evaluation services tosupport the hiring and selection process for Fire Department personnel. The Fire Departmentconducts academies approximately every one to two years and promotes or hires personnel asneeded. Academy sizes range from 8 to 24 recruits, depending on department vacancy levels.The District intends to establish a three (3) year, as-needed agreement with the Proposer whosequalifications, experience, and service approach best satisfy the requirements of the Scope ofService (Exhibit A). The Districtalso reserves the right to cancel or reissue the RFP in its sole discretion. If an amendment isissued, the District shall provide notice of the amendment to all Proposers. The intent is to assist respondents in the preparation of their submissions and toassist the District by simplifying the review process providing standards for comparison ofsubmissions.Proposals submitted in response to this RFP shall include a complete response to therequirements in this section in the order presented. The District discourages lengthy and costlyproposals. Proposals should be a straightforward delineation of the respondent’s capability tosatisfy the intent and requirements of this RFP and should not contain redundancies andconflicting statements.Proposal Format and ContentProposals should be prepared on standard 8 1/2" x 11" paper. All proposal pages should benumbered. Failure to follow this specified format, to label the responses correctly, or to addressall of the subsections may, at the District’s sole discretion, result in the rejection of the proposal.Proposals must contain the following information in the order listed:1. Introductory letterThe introductory letter should be addressed to:Carolyn BaptistaCosumnes CSD10573 East Stockton Blvd.Elk Grove, CA 95624The letter shall include the following:a. Proposer’s legal name and corporate structure.b. Proposer’s primary contact information, including name, address, phone, website,and email.c. Identification of use of subcontractors and scope of work to be performed bysubcontractors.d. Identification of any pending litigation against the Proposer.e. Disclosure of any bankruptcy or insolvency proceedings in last ten (10) years.f. Statement indicating the proposal will remain valid for at least 120 days.g. Signature of a company officer empowered to bind the Proposer to the provisionsof this RFP and any contract awarded pursuant to it.2. Executive SummaryThis section of the proposal should provide a concise synopsis of Proposer’s proposaland credentials to deliver the services sought under the RFP. In addition, it should explainhow the proposed solution will differentiate itself from other vendor solutions and thereasons why the District should select the proposed solution. This may include a list ofthe unique features that give the Proposer a competitive edge. The Executive Summaryshould not exceed three (2) pages.3Cosumnes Community Services District RFPPre-Employment Psychological Evaluation Services (FD-26-017)3. Qualifications and ExperienceDescribe the Contractor’s capability for undertaking and performing the work, includingany professional licenses and certificates held by the Contractor. List types and locationsof similar work performed by the Contractor in the last five (5) years that bestcharacterizes the quality and past performance.4. ReferencesThe Proposer must provide at least three (3) references which they have conductedsimilar services to within the last five (5) years. References may be contacted as part ofthe selection process. For each reference, proposer must provide the followinginformation:a. Name and contact information (i.e. name, title, address, phone, and email)b. Brief description of work provided.5. Work PlanThe work plan must state the Contractor’s ability to provide psychological evaluationservices. The plan should be simple, easy to read and follow, and address and satisfy theobjectives and specifications as listed in Section 3 – Scope of Work in this RFP.6. FeeThis section should include the cost for requested services outlined in Section 3 – Scopeof Work. Contractors shall clearly outline fees for the services to be provided. Uponcompletion of tasks, the Contractor will notify the District and request payment for theproducts and services by submitting an invoice and a brief description of work performedduring the billing period. Upon receipt of the invoice, the District will review in a timelymanner the products and services noted, verify completion, and authorize payment. Nocost increases shall be passed onto the District after the proposal has been submitted.7. Commitment to Diversity, Equity, and InclusionThe District is committed to ensuring that diversity, inclusiveness, and equity are integralparts of day-to-day management, work, and service delivery. Commitment to Sustainability and Environmental PracticesThe District has a commitment to conduct business with entities that follow sustainablepractices and provide safe and healthy workplaces for the individuals employed. TheProposer may state any programs they have implemented, as well as any environmentallysustainable practices.4
