ADDENDUM NO. 5

Dear Proponents,

This addendum is issued to reflect the following immediately. It shall be the responsibility of

interested Offerors to adhere to any changes or revisions to the IFB as identified in this Addendum

No. 5. This documentation shall become permanent and made part of the departmental files.

1.50 PROJECT RECORD DOCUMENTS

E. Contractor shall prepare record drawings information under the direction of a

Licensed Professional Surveyor. As-Built Record Drawings shall include elevation at

top of pipe, northing and easting of top of pipeline or new utility at intervals not to

exceed 250 feet and at all manholes, including rim elevations, invert in, invert out.

For drop manholes, as applicable both invert in pipes shall be provided. Contractor

shall provide as-built survey of the newly constructed access road, not to exceed every

100 feet. All culvert crossings shall be surveyed at the beginning to end. Ties to surface

features for triangulation purposes in the field shall also be included. Final As-Build

Record Drawing shall be stamped by a Licensed Professional Surveyor, tied to

established control monuments and other reference points (including D.O.T.

monuments and mile markers if available) on the New Mexico State Plane Coordinate

System, Central Zone, NAD83/NAVD88 datum, stating combined ground-to-grid scale

factor used, equipment used and date of completion of survey.

The bid quantities #9, 10, and 11 for the manholes, the description includes,

“interior epoxy coatings.” Please provide the specifications for these epoxy

coatings.

The inclusion of “interior epoxy coating” shall be struck and removed from the

bid item description for Bid Items #9, 10, and 11.

I would like to clarify whether Bid Items #41 through 62 are intended to be

part of an alternate. This is not specified in the bid schedule, but at the end it

requests two total amounts, which makes it unclear.

Bid Items #41 to 62 from Addendum No. 3 for the Colibri Sewer Re-Route are

part of the base bid and should not be considered alternates. An updated bid

form was released in Addendum No. 4, which did include alternates for the

pipe material through the existing casing on sheet C-25R (released in

Addendum No. 4).

Additionally, the Bid Form was restructured to distinguish between bid items

and alternates, and to clarify how the total project cost is to be calculated with

both components included.

Please add this Addendum No. 5 to the original proposal documents and refer to proposal documents,

hereto as such. This and all subsequent addenda will become part of any resulting contract documents

and have effects as if original issued. All other unaffected sections will have their original

interpretation and remain in full force and effect. Responders are reminded that any questions or need

for clarification must be addressed to Tammy Jim, Procurement Specialist Senior at
