The purpose of the grant is to build stronger rural health networks, improve technology and shared operations, strengthen the rural health workforce and ultimately ensure that Vermonters receive the right care at the right time for an affordable cost. Through this Request for Proposal (RFP) the Agency of Human Services, Department of Vermont Health Access is seeking to strengthen Blueprint’s capacity to advance high-performing primary care practices that deliver coordinated, patient-centered, and population-focused care. This RFP details the proposal submission and evaluation processes, the required experience of the bidder, the scope of work to be performed, and the resulting contract deliverables.
