Notes	The Scope and Purpose of this Request for Proposal is to acquire the following: · Administrative services for a self-funded short term disability plan, · Fully insured Basic Life and AD&D and Voluntary Basic Dependent Life and AD&D Insurance · Fully insured Voluntary Life Insurance for Employee, Spouse and Children · Fully Insured Voluntary Accidental Death and Dismemberment · Fully Insured Long Term Disability Insurance. Clear Creek Independent School District, "the District," intends to initiate this contract for an initial one-year period commencing on September 1, 2026 with annual renewals requiring approval of the Board of Trustees for up to an additional 4 years. The District expects to award this contract after approval by the Board of Trustees at the May 18, 2026 regular meeting. Service and cost price comparisons will be made as required using the terms and conditions of this proposal.
