Termitesand carpenter ants are included on an ‘as needed’ basis.Areas that may require service are common areas, hallways, maintenance areas, offices, kitchens, storage areas,dining rooms, bedrooms, bathrooms, showers, closets, crawl spaces, basements, elevators, loading docks and anyother areas as specified by DMMHA. Buildings may or may not be occupied. See the bid forms in section 6 of thissolicitation for information regarding building locations and size.NOTE: CONTRACT KICK-OFF CONFERENCE: Upon execution of the contract, but before treatment program start,the selected contractor and the DMMHA Representative shall hold a contract kick-off meeting to discuss project roll-out, project requirements, establish an approximate project schedule, and review specific site location details.Page 3 of 31 pagesDeadline: 3/6/2026 12:00 PM (CT)W26-1782 BackgroundThe Des Moines Municipal Housing Agency (DMMHA) provides Public Housing rental units for qualified low-incomefamilies and administers the Section 8 rental assistance program for Polk County. These programs are funded bythe U.S. Department of Housing And Urban Development (HUD) and administered by DMMHA, the Housing ServicesBoard, and the Municipal Housing Governing Board (Des Moines City Council).To assist the DMMHA with its mission to provide quality affordable housing opportunities within Des Moines and PolkCounty, the agency requires reliable, professional service from contractors licensed in the pest control industry toimplement an effective Integrated Pest Management (IPM) program utilizing scheduled and as-needed services.5 Apartment Complexes (see Exhibit B for floor plans)10 Single Family Houses (2-, 3-, 4-, and 5-bedroom homes scattered throughout the Des Moines area)Scheduled Services: See below requirementsAs-needed Services: Based on historical service requirements, the DMMHA estimates the following needs:Houses: 15 calls to contractor annually – each call requiring 2 service visits (all service types combined)Apartments: 50 – 60 bed bug infestations confirmed annuallyNote: Award of a contract does not guarantee a minimum or maximum amount of work. Historical data, quantityestimates and /or projections are provided only as a courtesy to assist bidders with assessing the probable cost ofthe work and/or for the City’s comparison/evaluation of bids submitted.Note: See floor plans in the "Attachments" tab for the five (5) apartment complexes.3 DefinitionsFor the purposes of this solicitation, the following definitions apply unless otherwise explicitly stated or relating tothe City of Des Moines Procurement Officer and staff assigned to assist with this RFP:wherever the term "City" is used, it is understood to mean the Des Moines Municipal Housing Authority(DMMHA);wherever the term "City Council" is used, it is understood to mean the DMMHA Board;I acknowledge and understand the above definitions(Required: Check if applicable)4 No Contact PolicyAny contact initiated by the Proposer or by a City representative, other than the Procurement Divisionrepresentative listed herein, concerning this Request for Proposal is prohibited. Any such unauthorized contact maycause the disqualification of such Proposer’s proposal from acceptance or consideration.Following the Proposal submittal deadline, Proposers shall not contact the Procurement Division or any otherCity employee or representative except to respond to a request by the Procurement Division.I acknowledge and agree to the above(Required: Check if applicable)5 Federal RequirementsThe Proposer agrees to the comply with all applicable the federal requirements, including the documents in the"Attachments" tab (HUD 5369-A, HUD 5369-B, HUD 52158 and HUD 5370-C) and any others determined necessaryby the City before the contract is awarded.I acknowledge and agree to the above.(Required: Check if applicable)Page 4 of 31 pagesDeadline: 3/6/2026 12:00 PM (CT)W26-1786 Statement of WorkThe selected contractor shall be required to furnish all supervision, labor, materials, tools, travel, required permits,licenses, and equipment necessary to complete the work as specified. The work involved shall include, but not belimited to, the following: control of rodents, crawling and flying insects, cockroaches, birds, and bed bugs. Termitesand carpenter ants shall be included on an ‘as needed’ basis.Pest control services shall be completed by professional grade liquid, aerosol, gel, and/or dusting, snap traps, androdenticides.Services expected and required include, but are not limited to:1. Snap and/or sticky traps for rats and Mice2. Rodenticides: When used shall be in tamper-resistant plastic boxes3. Cockroaches:i. Presence will be monitored in each unit with at least three sticky trap monitors in each unit.ii. Boric acid or desiccant dust and baits shall be used at unit turnover for cockroach control, within 72hours.iii. The type of treatment will depend on the level of infestation; low, medium, or high, with low and mid-level infestations receiving treatment with gel and vacuuming.4. No sprays or foggers shall be used by contractors without the advance approval of DMMHA.5. PESTICIDES LIMITED: Use pesticides only as necessary, with preference to products that while producingthe desired level of effectiveness, pose the least harm to human health and the environment6. Rodent control shall be done by bait stations strategically located in probable problem areas and shall notbe placed visible to the public, except for outdoor bait stations that cannot be hidden from public view.7. Termites & Carpenter Ants: Contractor shall notify the DMMHA Representative upon discovery.8. Bed Bug Inspection and Treatment: see "Attribute" #12 – Bed Bug Service RequirementsI have read the Service Requirements(Required: Check if applicable)7 Frequency and Schedule of ServicePest Control services shall be conducted during the working hours of 8:00AM to 5:00PM, unless otherwiseapproved by DMMHA.Scheduled services shall be as follows:Exterior Treatment of the 5 Apartment Buildings - Minimum of 1 time per monthHouses - On Call/As needed basis onlyTreatment for general pests/rodents (apartments - rotating basis, 1/3 of units treated per month in eachbuilding)Bed Bug Inspections (apartments) - within 30 days of contract execution (canine inspect all units) in eachbuildingBed Bug Inspections (apartments) - rotating basis, 1/3 of units canine inspected per month in each buildingNote: A chemical barrier application shall be completed in units where the canine inspection is conducted.Note: Canine must be certified by third party certification for bed bug detection, such as NESDCA, WDDO orIBBMA.Note: See "Attribute" #12, Bed Bug Service RequirementsPage 5 of 31 pagesDeadline: 3/6/2026 12:00 PM (CT)W26-178
