INTRODUCTIONOgden City desires to obtain proposals from qualified entities to provide the necessary labor andmaterials for pest control services at various Ogden City Facilities.A. Goals/ObjectivesOgden City has several facilities that will be placed on regular treatment schedules. Otherfacilities will be treated on an as needed basis. Proposals are being solicited fromexperienced contractors to provide the necessary labor and materials for pest control.B. Contracta. Award - It is anticipated that this solicitation will result in the award of one (1)contract; however, the City reserves the right to make multiple awards ifdetermined to be in the best interest of the City.b. The RFP document and the successful Proposer’s response will be incorporatedinto and become part of the final contract.c. Term and Pricing – Unless otherwise specified in the Scope of Work, the initialcontract term shall be three (3) years from the effective date of theagreement. Upon mutual written agreement of both parties, the contract may berenewed for one (1) additional one-year term extension, for a maximumcontract term of four (4) years.i. Proposers shall guarantee pricing for the initial contract term.d. Contractor is expected to maintain all required licenses, certifications,insurance coverages and endorsements in accordance with the contractrequirements.e. Renewal - The City reserves the right to review pricing, market conditions, andcontractor performance prior to exercising any renewal option. Renewal is notguaranteed and shall be at the sole discretion of the City.Pest Control Services RFP 2026 | 3II. SCOPE OF WORK or SPECIFICATIONS – Refer to Exhibit AIII. RESPONSE TO RFPOgden City is seeking proposals from providers capable of providing all the workdescribed in the Scope of Work including attachments.A. Each Proposal must include, as a minimum, the following information:1. Authorized Representative – Indicate name, address, email and telephone numberof the company submitting the proposal.i. Include the name and contact information of the person designated asauthorized to contractually bind the offer.2. Company Experience - A description of the firm’s experience and capability offulfilling this contract if awarded.i. Include company history with biographies and/or resumes for principalcontacts.3. Team Information – Provide the names of any outside consultants and/orsubcontractors to be utilized, including contact information and a briefdescription of their role(s) in providing pest control services.4. Exhibit B, Cost Proposal – Completed form to provide detailed breakdown of theproposed costs including service guarantee, retreatment policies, response times,and warranties offered under your proposal. If more space is needed, includeinformation in company letterhead. Prices need to be guaranteed for the initialcontract term.5. References – Provide list of at least three references; include project dates, scope,summary of work performed, and contact information.6. Evidence of Insurability – COI, Refer to Ogden City’s insurance requirements (asapplicable).7. Copy State of Utah Contractor’s License for performing commercial pest controlservices.B. Proposals are to be no longer than 20 pages. Double-sided pages count as two pagesPest Control Services RFP 2026 | 4C. For City record-keeping purposes, please do not use spiral or wire binding methods. Thefollowing methods will be accepted:1. Submitted as loose leaf with binder clip2. Submitted in a regular 3-ring binderD. Proposals submitted to Ogden City are considered public records, unless protected withinUtah Code 63G-2-1.IV. EVALUATION OF PROPOSALSProposals will be evaluated in accordance with the criteria listed below:A. Authorized Representative InformationB. Evidence of InsurabilityC. List of Client ReferencesD. Capability and experienceE. Availability and response timeF. Cost / Fee proposal / Service Guarantee & WarrantyPass or FailPass or FailPass or Fail30 Possible Points30 Possible Points40 Possible PointsA total of 100 possible points may be awarded to one proposal.The selection committee will primarily be composed of City employees. On occasion,consultants may be invited to participate in the review.Note that proposals that are received after the deadline or not conforming to the RFPrequirements may be deemed non-responsive and eliminated. Each Proposer bears soleresponsibility for the items included or not included in the response submitted by thatProposer.All proposals in response to this RFP will be evaluated in a manner consistent with theOgden City policies and procedures. Ogden City reserves the right to disqualify anyproposal that includes significant deviations or exceptions to the terms, conditions and/orspecifications in this RFP.
