This Addendum is issued to modify or clarify the bid documents previously furnished for the

above-referenced project. The following changes shall be incorporated into the contract

documents:

Plan Revisions:

1) Sheet 3 “Typical Section No. 2”: Revised to correctly identify Asphalt Concrete Type

S9.5C for maps 10 & 11 Please replace the original sheet 3 with the attached revised sheet.

2) Sheet 6 “Summary of Quantities”: Revised to update the quantities for the following items:

• Asphalt Concrete Type S9.5C: Quantity reduced on Map 7.

• Asphalt Binder for Plant Mix: Quantity reduced on Map 7.

Please replace the original sheet 6 with the attached revised sheet.

Proposal Revisions:

1) Pages 99 - 100 “Bid Form”: Revised to match the changes in the summary of quantities.

Please replace the original Pages 99 – 100 with the attached revised pages.

2) Electronic Bid File: The amended EBS file (DA00667.001x) has been uploaded.
