Bid Title: Family Stabilization Program
Category: County General Status: Open Description: Lake County Behavioral Health Services (LCBHS) are pleased to announce the availability of up to $200,000 per year for three years (2026-2029) through the Behavioral Health Services Act (BHSA) to provide the Family Stabilization Program by providing residential and community based intensive case management, transitional housing for young (18-25) pregnant or parenting families and their children, and coordinated service linkages.
Participants served under this program must meet BHSA Housing Interventions eligibility criteria, including applicable mental health and/or substance use disorder (SUD)–related eligibility requirements.
Local applicants must submit their proposals to LCBHS no later than 5:00 pm on May 29th, 2026.
The contracting process, leading to funding disbursement, will begin immediately after an applicant has been given a letter of intent.
Proposals should be submitted in accordance with the requirements of this solicitation, which are contained herein.

Theprogram has provided Family Stabilization services by providing transitionalresidential services for young pregnant and/or parenting families and providingeducation and skills training for up to 15 months.Current Need: It has been recognized that there is an ongoing need to assistvulnerable young families in the community who are at risk or experiencinghomelessness. It is also recognized that these families can benefit from a youthdriven process to reduce risk factors and increase self-sufficiency. Theseprocesses can help to end the cycle of poverty for these vulnerable families andprovide long-term sustainable change that will impact consumers as well as thecommunity for years to come.Funding: Lake County Behavioral Health Services have determined Housingfunds from the Behavioral Health Services Act (BHSA) are for the operation of theFamily Stabilization Program. LCBHS is making available up to $200,000 perfiscal year for a period of up to three (3) years, contingent upon:• Annual appropriation by the County• Continued availability of BHSA funding• Contractor performance• Ongoing alignment with the approved BHSA Integrated PlanLocal applicants must submit their proposals to LCBHS no later than 5:00 pm onMay 29th, 2026.The contracting process, leading to funding disbursement, will begin immediatelyafter an applicant has been given a letter of intent.3|PageFamily Stabilization ProgramProposals should be submitted in accordance with the requirements of this solicitation,which are contained herein.2. SCOPE OF SERVICE.The selected subrecipient shall design, implement, and operate a Family StabilizationProgram serving pregnant and parenting Transitional Age Youth (TAY), ages 18–25, andtheir children who are experiencing or at risk of homelessness and who meet BHSAHousing Interventions eligibility criteria, including applicable mental health and/orsubstance use disorder-related eligibility requirements.A. Transitional Housing• Provide time-limited safe, stable, low-barrier transitional housing for eligibleparticipants and their children• Maintain facilities in compliance with all applicable local, state, and federalrequirements• Ensure housing is delivered consistent with Housing First principles,without preconditions for service participationB. Supportive Housing Services• Deliver individualized, trauma-informed, and strengths-based services• Conduct comprehensive assessments and develop individualized housingstability plans• Provide ongoing support focused on housing retention, self-sufficiency, andfamily stabilityC. Family Stabilization Services• Provide parenting support, life skills training, and household managementeducation• Support participants in developing independent living skills and long-termstability• Deliver or coordinate behavioral health and wellness services, as appropriateD. Service Coordination and Linkages• Establish and maintain partnerships with community-based organizations andhealthcare providers• Facilitate access to:o Behavioral health and primary care serviceso Public benefits and income supportso Employment and education services• Execute Memorandums of Understanding (MOUs) to support a coordinatedsystem of care4|PageFamily Stabilization ProgramE. Housing Navigation and Permanent Housing Placement• Assist participants in identifying and securing permanent housing includinglandlord engagement• Provide housing search assistance, landlord engagement, and applicationsupport• Support transitions from program participation to permanent housingplacementsF. Coordinated Entry System (CES) Participation• Actively participate in the Lake County Coordinated Entry System• Accept referrals and prioritize clients in alignment with CES policies• Participate in case conferencing and housing prioritization processesG. HMIS Data Collection and Reporting• Enter and maintain accurate, timely client-level data in HMIS• Track and report required performance metrics, including:o Number of persons servedo Housing outcomes and exits to permanent housingo Returns to homelessnesso Income growth and service utilization• Participate in Housing Inventory Count (HIC) and other required reportingH. Equity and Culturally Responsive Services• Provide culturally responsive, trauma-informed, and inclusive services• Implement outreach strategies to engage underserved and disproportionatelyimpacted populations• Address barriers to access and promote equitable outcomesI. Program Administration and Compliance• Ensure all activities comply with BHSA requirements and applicable federaland state regulations• Maintain required policies, including confidentiality, grievance procedures,and code of conduct• Ensure coordination with LCBHS, the Lake County Continuum of Care,Partnership HealthPlan, Enhanced Care Management providers, CommunitySupports providers, and other applicable partners to prevent duplication ofservices and ensure BHSA funds are used only for allowable costs nototherwise reimbursable through Medi-Cal, MCP Community Supports, orother available funding sources, unless expressly authorized by LCBHS.• Participate in LCBHS monitoring, site visits, and audits• Submit required quarterly and annual program and expenditure reports5|Page
